We're looking for a Clinical Care Coordinator to lead our clinical care initiatives, specifically overseeing the Skin and Wound Program and admissions at our 166-bed long-term care facility. Key Responsibilities:
Admissions & Coordination:
- Lead and coordinate the admissions process, ensuring smooth transitions for new residents.
- Maintain occupancy rates above 97% through diligent tracking and timely responses to Ontario Health At Home.
- Collaborate with social workers, families, and the clinical team to facilitate seamless admissions, discharges, and transfers.
- Prepare and complete necessary documentation, assessments, and ensure that new residents' rooms are ready and welcoming.
- Support families during the admissions process by explaining care plans and other essential details.
Skin & Wound Care Program:
- Oversee and assess residents' skin and wound care needs, ensuring high standards of care.
- Lead wound rounds and ensure that the latest skin care treatments and best practices are implemented.
- Train and educate staff on skin and wound care policies, and track progress with monthly reports.
- Coordinate with dermatologists and specialized wound care teams to provide comprehensive care to residents.
Leadership & Team Management:
- Monitor the performance of nursing staff, provide leadership, and help identify areas for staff development.
- Foster a culture of teamwork and accountability, ensuring the highest standards of care are delivered consistently.
- Ensure that staffing levels meet the needs of the unit and that care is always provided with empathy and professionalism.
Quality Improvement & Safety:
- Participate in ongoing quality improvement initiatives and ensure compliance with relevant health and safety regulations.
- Lead by example in promoting a safe and healthy environment for both residents and staff.
- Work with the leadership team to assess needs and implement changes that enhance care delivery.
Qualifications:
- RPN with current Certificate of Competence from the College of Nurses of Ontario.
- Wound Care Management Certification is an asset, or experience in wound care management.
- Two years of experience in long-term care, with a strong understanding of nursing practices and resident care needs.
- Familiarity with Substitute Decision Maker’s Act and Capacity Assessments is a plus.
- Current CPR Certification required.
- Strong communication, organizational, and leadership skills.
- A valid Vulnerable Sector Check for Employment completed by your local Police Department - No older than 6 months
- Two-step TB test or chest x-ray - No older than 6 months *this has to be fill out within 14 days of hire*
